Wave 1 (2024) Expression of Interest

Data Modernization Implementation Center Program

We invite expressions of interest from SAM-registered state, local, territorial, and freely associated state public health agencies (PHAs). A complete project plan is not required to submit an Expression of Interest (EOI). Please note, agencies that received funding previously via the Public Health Infrastructure Grant (PHIG) are eligible to submit an EOI and may be eligible for funding. Tribal public health authorities will be eligible to apply for support from a Tribal Implementation Center (IC) once this IC has been established. 

Program Details

The Implementation Center Program complements previous investments made through PHIG, a major federal investment supporting U.S. infrastructure needs.

Duration and Timeline

The EOI will be open on a rolling basis through Year 1 of the program. Agencies that submit EOIs by October 15, 2024 will have priority consideration for the first wave of the program.

PHAs selected for the initial wave of the Implementation Center Program can expect to be notified via email by November 4, 2024. Notifications will be sent to the contact(s) provided in the EOI form.

Upon notification, selected agencies will receive detailed instructions on the next steps. The agencies will attend a facilitated assessment with an Implementation Center to identify existing resources and specific needs. This assessment will help tailor the support and resources provided to adopt health IT standards and participate in data exchange networks.

Eligibility Criteria

Participant Requirements

State and Territorial PHAs will be asked to provide a letter of support from the State or Territorial Health Official as part of their EOI submission. Local PHAs will be asked to provide an attestation that their agency has notified their State or Territorial Health Official and/or DMI Director that they plan to submit an EOI for this Program.

Qualifications and Experience

PHAs submitting an EOI will be asked to identify a potential project that aligns with the IC Program Outcomes and ability to participate in the Program. If EOI criteria are met, PHAs will undergo a Facilitated Assessment with an Implementation Center to dive into more details on PHA potential projects, capacity, and sustainability plans. These assessments will inform selection to receive support for a PHA project. If not selected to receive support for a PHA’s IC project, then the IC Program will offer community support.

IC Project Support: Assistance offered to PHAs participating in the IC Program to implement data infrastructure enhancements includes:

  • Implementation assistance may cut across four key domains (People, Process, Governance, and Technology).
  • Financial assistance through grants or other funding mechanisms aligned with the selected use case.
  • Sustainability planning to maintain gains that continue beyond the IC Program.

Community Support: Assistance offered to PHAs outside of direct IC project support and available to the broader public health community. These activities may include the dissemination of lessons learned from implementation projects via webinars, communities of practice, and playbooks.

Confidentiality

All information provided in this EOI form will be treated as confidential and will be used solely to engage participants for the Public Health Infrastructure Grant National Implementation Center Program. CDC, ASTHO, NNPHI, and PHAB are committed to ensuring the privacy and security of information shared by public health agencies.

Information submitted in this form will only be accessible to authorized personnel involved in the assessment and selection process. It will not be disclosed to third parties without the explicit consent of the submitting agency, except as required by law. Data provided will be stored securely and used strictly in accordance with applicable data protection regulations.

Legal and Compliance

Compliance Statement:

By submitting this Expression of Interest (EOI) form, your public health agency agrees to comply with all applicable laws, regulations, and policies governing CDC grant funding. This includes, but is not limited to, compliance with federal, state, and local laws regarding data privacy, security, and public health reporting.

Data Privacy and Security:

The Centers for Disease Control and Prevention (CDC), the Association of State and Territorial Health Officials (ASTHO), the National Network of Public Health Institutes (NNPHI), and the Public Health Accreditation Board (PHAB) are committed to protecting the privacy and security of all data submitted as part of this program. Information provided in this EOI will be used solely for program evaluation and selection and will be handled in accordance with applicable data protection laws and regulations.

Non-Discrimination:

Participation in the Public Health Infrastructure Grant National Implementation Center Program is open to all eligible public health agencies without regard to race, color, religion, sex, national origin, age, disability, or genetic information. The program adheres to the principles of equal opportunity and affirmative action.

Reporting and Accountability:

Selected public health agencies will be required to engage in regular reporting and evaluation activities as part of their participation in the program. This includes providing feedback on the program’s impact and outcomes.

Termination or Withdrawal:

The CDC, ASTHO, NNPHI, and PHAB reserve the right to terminate or withdraw an agency’s participation in the program for non-compliance with program requirements or other legal obligations. Participants may also withdraw from the program at any time by providing written notice to phdmic@astho.org. By completing and submitting this EOI form, your agency acknowledges and agrees to the terms outlined in this Legal and Compliance Statement.
